The Clinical Pharmacist, Pharmacy Outreach (internally: Pharmacist, Pharmacy Outreach) is responsible for optimizing patient outcomes through telephonic interventions. This role focuses on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization.   Candidates should be comfortable working remotely from anywhere within the US, across all US time zones.     Please note: Valid pharmacist license in at least 2 of the following markets and a willingness to acquire more states as needed:    AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE
